New Patients

To become a new patient with Northland Neurology, we must receive a referral from your provider.

During your first visit we will obtain important background information such as your medical history and insurance information. On this page you will find a map and directions to our office, practice hours, and payment policies. You can even save some time at your first visit by printing out and completing the patient forms to bring to your appointment.
